2025–2026 Provider Profiles
Are Now Available in the Provider Portal!
We are excited to announce that the 2025 Provider Profiles are now live in the Provider Portal. To complete your 2025 Provider Profile, please follow these steps:
- Log in to your Provider Portal account.
- Complete 2024 – 2025 Cost of Care Survey (if not already completed)
- Open your current Profile and click the blue “Create 2025” button at the top.
- Review each section of your Profile for accuracy and make any necessary updates.
Please note that most of your current information will carry over automatically, except for your calendar closure dates. As you review and update your Profile, keep the following in mind:
- If you plan to increase your rates for the 2025–2026 program year (effective July 1, 2025), be sure to:
- Update the “Private Pay Rate” tab.
- Use the SR daily rate helper button at the bottom of the screen.
- Upload a new rate sheet using the updated template (Download Here) with an effective date of July 1, 2025.
- Once all sections are updated, submit your Profile for Coalition review.
Documents
To remain compliant with your SR and/or VPK contracts, ensure that all required documents are valid. If any documents expire before June 30, 2025, please upload updated versions as soon as possible.
- W-9: Upload a signed copy dated 2025 (Download Here)
- E-Verify: Upload a fully completed, signed, and notarized form for 2025–2026 (Download Here)
- Sunbiz: Upload the latest annual report.
Important Deadlines
All Provider Profiles must be completed and made Active (with no further revisions needed) by April 30, 2025.
For your convenience, we’ve attached a list of holidays and important dates for the 2025–2026 program year (Download Here). You may choose to select these as SR Paid Holidays (to be marked on the Closures Calendar) and VPK Non-Instructional Days (in the VPK APP).
Reminder: While you can choose any number of closure days for your program, only up to 12 days can be included in your SR contract for reimbursement.
